Terms and Conditions of Water Supply Application
  1. All applications must be submitted by licensed plumber who are registered with Suruhanjaya Perkhidmatan Air Malaysia (SPAN) to application / deposit unit.
  2. Application form that needs to be filled in is form SYA/F1A/A or SYA/F1A/B or SYA/F1A/C. Completed from must be signed by the applicant and follow the terms and conditions of water supply application.
  3. Checklist of water supply application by category is as follows:
     
    a. Housing Scheme (Domestic Individual Meter)
   
• 
Completed form of SYA/F1A/A
   
• 
Accept the Water Supply Application Terms and Condition
›  SYA/T1/1 for landed property
›  SYA/T2/1 for individual sub meter which water supplied through Bulk Meter
   
• 
Copy of applicant's Identification Card (I/C)
   
• 
Approval Plan File No. from SYABAS accompanied by a copy of Approved Internal Plumbing Plan
   
• 
Applicant's name and address
   
• 
A site plan that indicates the layout of the proposed site, complete with the house number, road and area that has been approved by Local Authority (P.B.T)
   
• 
Water supply system that has been approved and submitted to SYABAS.
       
    b. Land with Grant / Temporary Occupancy License (TOL)
   
• 
Completed form of SYA/F1A/A or B or C (depending on usage of the premise)
   
• 
Copy of grant indicating the owner's name and grant number / T.O.L tax for the current year (For areas under Local Authority (P.B.T) administration, refer to District Office / Local Council for rules and regulations)
   
• 
Copy of applicant's Identification Card (I/C)
   
• 
Copy of water bill of the nearest premise to the proposed site (if available)
       
    c. Stalls and Squatters

When will your water meter be installed?
  As soon as your application has fulfilled the terms and conditions as stated and you would have obtained the Certificate of Fitness (C.F) or Temporary Certificate of Fitness (T.C.F) for properties (within the local authority only). Also, all related bills have been settled.
   
     
Application to Change Name and Account of Consumer
  Change of name and consumer's account takes place when there is a change of ownership/occupant on a premise
    1. Close of Account
Download Form SYA/F8/2 (Borang Penutupan Akaun dan Permohonan Pulangan Balik Cagaran)

 Terms and Conditions to Close an Account:
   
• 
Form SYA/F8/2 (Borang Penutupan Akaun dan Permohonan Pulangan Balik Cagaran) must be filled-in and signed by current account owner
   
• 
All outstanding balance must be settled before closing an account
   
• 
Original copy of water deposit receipt must be handed to SYABAS for the purpose of returning deposit. If it is not available, statutory declaration must be submitted.
       
    2. Change of name and account
   
Form SYA/F1A/A must be filled-in and signed by new account owner
   
Copy of related documents (such as Sale and Purchase Agreement, Land title, Quit rent or Tenancy/Lease Agreement, etc.)

›  SYA/T1/1 for landed property
›  SYA/T2/1 for individual sub meter which water supplied through Bulk Meter
   
Water bil of the premise (if available)
   
Copy of Identification Card (I/C) or Company Registration Number and Company Chop/Stamp
     
     
Why water supply disconnection is enforced?
 
To collect outstanding balance of consumers' bills
 
To increase cash flow efficiency
 
To create awareness among consumers on the importance of settling water bills within the given period of time
   
     
Types of Water Supply Disconnection
  Water supply disconnection is categorized into three (3) types i.e. :
 
Disconnection for non payment / enforcement
 
Disconnection on request
 
Technical Disconnection
   
     
Disconnection for non payment / enforcement
  It will be carried out when consumer :
 
Fails to settle water bill within the given period of time
 
Fails to settle outstanding balance
 
Unlawfully diverts or takes water from any water supply system or part thereof
     
   
Disconnection on request
  Consumers may apply to close their accounts on one of these reasons :
    •  Move to another house
    •  Premise collapsed / destroyed by fire
  Consumers are required to submit the following documents :
     
Documents needed to close account on request
 
 • 
Application letter
 
 • 
Form SYA/F8/2 (Borang Penutupan Akaun dan Permohonan Pulangan Balik Cagaran)
 
 • 
Original copy of water deposit receipt / Statutory Declaration
     
     
Technical Disconnection
 
(i)
SYABAS shall have the right to disconnect water supply to any premise contravenes or fails to comply with any provisions of the WSIA 2006. These include but are not limited to any of the following:-
 
(a)
    on the agreement with SPAN and KTAK, to reduce or cease to provide water supply if by reason of any circumstances there is insufficient water to enable the full quantity to be supplied to the said premise;
 
(b)
    who, having a contract with a water distribution licensee for the supply of water, allows or causes any such supply of water to be carried or conveyed or used outside such premises for purposes other than the purposes of the contract except for the extinguishment of a fire;
 
(c)
    misuse or wastes or causes or permits to be misused or wastes any water supplied to his premises;
 
(d)
    having agreed to use the water supplied for a particular purpose only, uses or causes or permits the use of water for a different purpose other than the extinguishment of a fire; or
 
(e)
    has contravened any provisions of this Act or its subsidiary legislation.
 
(ii)
Section 89 (3) of WSIA stated that before exercising its powers under subsection (1), the water distribution licensee shall give a written notice to the owner, management corporation or occupier of the premises or the consumer to remedy or rectify the default or contravention within fourteen days from the receipt of the notice.
 
(iii)
However, a 14 days notice in writing to the consumer or the occupier of the premise shall be issued by SYABAS stating its intention to disconnect the supply.
 
(iv)
The Chief Executive Officer shall be the ONLY authorized officer to issue instruction for Work Order for cases under Technical Disconnection.
     
     
How to get back your water supply that has been disconnected?
 
Make full payment of outstanding balance at SYABAS District Office
 
Make full payment of water supply reconnection according to size of meter :
 
(a) For meters sizes below 25mm
 
(i) Disconnection due to non-payment                                                            RM25.00
 
(ii) Reconnection on payment                                                                           RM25.00
 
(iii) Disconnection and reconnection on request                                          RM25.00
 
(iv) Remove plug at communication pipe to restore supply                        Actual cost plus 25%
 
(b) For meters sizes above 25mm
 
(i) Disconnection of 26mm to 50mm meter for non-payment                    RM75.00
 
(ii) Reconnection of 26mm to 50mm meter on payment                            RM75.00
 
(iii) Disconnection of 51mm to 150mm meter for non-payment               RM150.00
 
(iv) Reconnection of 51mm to 150mm meter on payment                         RM150.00
 
Make full payment of other related bills (if any)
     
     
How water supply is disconnected?
  Water supply is disconnected by using one of the following methods :
 
Seal
 
Clamp stop-cork
 
Remove meter and plug
 
Closing of the Integrated Lockable Valve (ILV)
 
Plug ferrule
   
   
When will your water supply be reconnected?
  Reconnection would be performed within one (1) working day after all the outstanding arrears and reconnection fee are settled.
   
   
Consumption Period and Bill Calculation
  Water charges are billed pro-rated for each tariff band based on a 31-day billing period and is only applicable to Domestic (Code 10) where there is a three band tariff and Commercial (Code 11) where there is two band tariff respectively.
 
  Example : Bill calculation for tariff code 10 (Domestic)
    Billing Days    - 33 days
    Billing Period  - 33 days/ 31 days (1 month) = 1.06 month:
    Total Consumption    - 40 m³

20 m³ x 33 days / 31 days 21.20 m³ x RM 0.57
RM 12.08
15 m³ x 33 days / 31 days 15.90 m³ x RM 1.03
RM 16.38
Remainder 2.90 m³   x RM 2.00
RM   5.80
TOTAL 40 m³
